Help us understand the property before the appraisal. Use this form to request information, ask about an appraisal, or provide details for an existing assignment. Include known improvements, system ages, additional structures, and supporting records. Complete what you know; leave optional items blank or enter "Unknown" rather than guessing.

Your request

Start here. Only the fields marked * are required.

Include city, state, ZIP code, and unit number. For property without an address, provide the assessor's parcel number (APN) and location. For a general inquiry, enter "Not yet identified."

Describe your question, intended use and intended recipient(s). Identify the requested value date separately from the requested report-delivery date. For an existing assignment, include the lender / client and file reference, if known. Do not include account numbers.

Property and access

Provide known facts and identify their source. Estimates are welcome when clearly labeled.

Provide original year built, bedroom and bathroom details by level, stories, known area figures and source, lot size, and APN / MLS number. Identify above-ground finished area, below-ground finished or unfinished area, conversions, and separate living areas individually rather than combining them. Describe bathroom fixtures when the bath type is unclear.

Sources of property information

Who should be contacted to arrange access, and at what phone number? Note preferred availability, occupied units, pets, restricted areas, attic / crawl-space access, locked outbuildings, and whether utilities are operating. Do not enter security, alarm, gate, or lockbox codes.

Updates and major systems

Select the areas you can describe. Include original or unchanged features, not just recent work.

Interior areas you can describe

Select all applicable. For each area, identify the unit / building and room or level. Original, unchanged, and unknown histories are useful too.

Exterior and systems you can describe

Provide approximate installation or replacement years when known. A receipt or permit is more useful than a guess.

Additional buildings and features

Select what is present or potentially present. Describe uncertain classifications rather than guessing.

Additional living areas and buildings

Leave unselected when not present. Note uncertain features in the applicable description; the appraiser will determine classification.

Site, utilities, and access rights

Include known conditions, available records, and any limitations on use.

Describe known private-road maintenance, shared driveways, easements, encroachments, land / ground leases, separate water / mineral rights, deed or use restrictions, association obligations not already noted, drainage / flooding, prior fire damage, or other site concerns. Note broadband availability and known waterfront / access rights or unusual views. Identify the information source; upload relevant records rather than drawing legal conclusions.

Condition and market information

Report what is known. The appraiser will evaluate significance and any required action.

Include anything not addressed above. Optional comparable suggestions should identify address, sale / listing date and status, price if verified, source / verification contact, and why the property may be relevant. Include off-market transactions or unusual concessions where known. Supply facts, not a requested minimum or target appraisal value.

Provide available records only. Do not obtain new inspections or reports solely to complete this form.

Up to 5 PDF, JPG, JPEG, PNG, or HEIC files in this field. All files in both upload fields combined must total 10 MB or less. Available items may include an improvement list, permits / final approvals, plans, surveys, title excerpts, solar agreements, association records, leases / rent roll, inspection records, or contracts and addenda. Redact unrelated sensitive information.
